Administration of Medication Form

Health Requirements for New Student Registration

Preschool and Kindergarten students are required to have a physical exam prior to their entrance to school. The physical exam, completed up to one year prior to the start of the school year, must include a height, weight, blood pressure, urinalysis, hematocrit or hemoglobin, scoliosis exam, assessment for tuberculosis risk and age appropriate immunizations (per Regulations of Connecticut State Agencies Section 10-204a-1).

Preschool through 8th Grade students entering Preston Public Schools from another Connecticut school district are required to have had a physical exam prior to entering Pre-School/Kindergarten and again after completing fifth grade, prior to entering 7th grade. Proof of age appropriate immunizations is required. A copy of these records should be obtained from the school the student is leaving and presented to Preston Public Schools upon registration.

Preschool through 8th Grade students entering Preston Public Schools from a school outside of Connecticut are required to have a physical exam prior to their entrance to school. The physical exam, completed up to one year prior to the start of the school year, must include a height, weight, blood pressure, urinalysis, hematocrit or hemoglobin, scoliosis exam, assessment for tuberculosis risk and age appropriate immunizations (per Regulations of Connecticut State Agencies Section 10-204a-1).

Medication in School

Students may receive medications in school according to these guidelines:

“Medication” is considered to be all precription medications as well as over-the-counter medications including but not limited to acetaminophen, ibuprofen, benadryl, cough and cold preparations, sunscreen, medicated chapstick and medicated/herbal cough drops.

An Authorization for Administration of Medication must be completed with a physician/dentist’s order for medication and signed by the parent/guardian.

Parents/guardians of students requiring medication may submit a completed Authorization for Administration of Medication form along with the prescription medication in a properly labeled pharmacy prepared container or, in the case of an over-the-counter medication, in the original packaging, to the school nurse. Students may not carry such medication to or from school.

In cases where a student is able to self administer an inhaler or automatic prefilled cartridge injector an Authorization for Administration of Medication form must be completed by the physician/dentist and parent/guardian indicating that self administration is authorized. Students may have these medications in their possession during school.

If a parent/guardian would like their child to have available cough drops with the active ingredient shown as “pectin” they may do so by providing the cough drops and a note giving such permission.
